Guidelines to use Patient Health Record: It is very important for anyone who downloads the Patient Health Record tool to take it to every physician appointment. You may want to place documents into a binder for ease of use. It is highly recommended for those using the PHR tool to get business cards for all physicians and other health professionals they see and insert them into the book/binder. Medication management is the most important part of the personal health record. Only current medications should be listed and the list should be updated any time a dose or drug change is made, to avoid confusion. We recommend members to put away any drugs not being used to ensure that they do not get mixed with the drugs that are currently being used. Chronic diseases and immunizations is a record the PCP can assist in checking off. It is very important for all physicians to know the chronic diseases and the current immunizations members have received. The Red Flag: The “Call Your Doctor” portion of this section of the binder is the second most important area of the personal health record. This is where a member writes down the symptoms his/her doctor identifies as Red Flags for other doctors who care for the member. These Red Flags enable preventative measures to be taken more quickly, resulting in happier, healthier members. The calendar is used for keeping track of all medical appointments. At the end of the calendar is a page for members to write questions they wish to ask their physician regarding their health care and the other page is for them to document the changes the physician recommends.
